Uroflowmetry (UFM) is a diagnostic method for urological diseases based on the measurement of various parameters during urination. The procedure is affordable and allows the detection of impaired urine outflow from the bladder.
This is a common problem among older men. It is very often associated with prostate diseases: if the prostate enlarges, the urethra becomes obstructed. Together with cystometry and bladder ultrasound, uroflowmetry is included in the comprehensive urodynamic study to assess the functions of the urinary system.

Indications for uroflowmetry

The indications for the procedure are problems with the genitourinary system, particularly with urination — it may be painful, intermittent, and difficult. Uroflowmetry is an important step towards establishing an accurate diagnosis.

The procedure is often prescribed when the following conditions are suspected:

Urethral stricture (narrowing of the urethra) – most often caused by trauma.

Prostate adenoma (benign prostatic hyperplasia) – a benign tumor that compresses the urethra.

Prostatitis (inflammation of the prostate gland) – enlargement due to inflammation compresses the urethra and interferes with normal urination.

Urolithiasis (urolithic disease) – stones formed in the bladder or kidneys may block the urinary outflow.

Sclerotic lesions of the bladder – scarring of the bladder neck occurs.

Neurogenic bladder – dysfunction of voluntary urination due to impaired neural regulation.

Tumorous processes – after the age of 50, the risk of such conditions in men is very high. Contributing factors may include harmful habits, hormonal imbalance, prostate adenoma, or prostatitis.

Neurological disorders caused by stress and trauma.

Preparation for the examination

Preparation consists of two simple stages:

Urine preparation. The bladder must contain at least 150 ml of urine. To achieve this, the patient should drink 1 liter of water one hour before the examination and refrain from urination until the procedure. The test is performed when the physiological urge to empty the bladder arises.

Psychological readiness. Since urination in a medical setting may create a psychological barrier, it is important for the patient to concentrate and follow instructions. Uroflowmetry is carried out in a private room where no one except the patient is present.

Procedure

Uroflowmetry is performed in a natural position: standing for men, sitting for women.

Steps of the procedure:

The patient is taken to a special room and introduced to the device. Five seconds after pressing the “start” button, the patient must urinate into a special funnel. The device, connected to a computer, records the urine flow rate in milliliters per second. During the procedure, the patient should remain calm and relaxed.

When urination is complete, the patient waits 5 seconds and then turns off the device.

The computer analyzes the data and produces a flow curve, which is interpreted by the physician. These results show the degree of urinary obstruction. Normal values differ by age and sex. For example, in a healthy man, the urine flow rate is 10–15 ml/sec, and complete bladder emptying takes about 20 seconds.

To complete the clinical picture, bladder ultrasound is performed to determine the residual urine volume, which should not exceed 30 ml. Based on the results, further diagnostic steps are determined. To establish an accurate diagnosis, a comprehensive urodynamic study is required, including dynamic uroflowmetry, bladder ultrasound, cystometry, and laboratory tests.
