This is a common problem among older men. Very often, it is associated with diseases of the prostate gland: when it enlarges, the urethra becomes obstructed. Along with cystometry and bladder ultrasound, uroflowmetry is included in the comprehensive urodynamic study for the assessment of urinary system functions.
Indications for uroflowmetry
The indications for the procedure are problems related to the genitourinary system, particularly urination—it may be painful, intermittent, and difficult. Uroflowmetry is an important step towards making an accurate diagnosis. The procedure is often prescribed when the following conditions are suspected:
Urethral stricture (narrowing of the urethra). The main cause is trauma.
Prostatic adenoma. This is a benign tumor that compresses the urethra.
Prostatitis. Inflammatory processes in the prostate cause its enlargement, compressing the urethra and interfering with normal urination.
Urolithiasis. Stones formed in the bladder or kidneys can block the urinary outflow.
Sclerotic lesions of the bladder. In this case, scarring of the bladder neck develops.
Neurogenic bladder. This is a dysfunction of voluntary urination caused by impaired neural regulation of the bladder.
Neoplastic processes. After the age of 50, the risk of this condition in men increases significantly. Causes may include harmful habits, hormonal imbalance, prostatic adenoma, or prostatitis.
Nervous system disorders caused by stress or trauma.
Preparation for the study
Preparation consists of two simple stages:
Urine preparation. At least 150 ml of urine must be accumulated. To achieve this, the patient drinks 1 liter of water one hour before the study and does not urinate until the procedure. It is performed when the physiological urge to void arises.
Mental preparation. This simple manipulation often poses a psychological barrier, as patients are used to urinating in designated places. Therefore, during the procedure, the patient needs to concentrate and perform the required actions. Moreover, uroflowmetry is carried out in a separate room where no one except the patient is present.
Procedure
Uroflowmetry is performed in the patient’s natural position: standing for men, sitting for women. The stages of the procedure:
The patient is taken to a special room and introduced to the device. Five seconds after pressing the “start” button, the patient urinates into a special funnel. The device is connected to a computer and records the urine flow rate in milliliters per second. During the procedure, the patient must remain calm and relaxed.
After urination is complete, the patient waits 5 seconds and turns off the device.
The computer analyzes the data and generates a graph, which is interpreted by the physician. These data determine the degree of urinary obstruction. Norms vary by age and sex. For example, in a healthy man, the urine flow rate is 10–15 ml/s, and the complete bladder emptying time is about 20 seconds.
For a more complete clinical picture, a bladder ultrasound is performed to assess the residual urine volume, which should not exceed 30 ml.
Based on the procedure, decisions are made regarding further diagnostics. To establish an accurate diagnosis, comprehensive urodynamic evaluation is necessary: uroflowmetry performed dynamically, bladder ultrasound, cystometry, and laboratory tests.
